RAI/MDS Coordinator



Job Description:Our client is in search for RAI/MDS Coordinator for their long-term care facility in London, ON. The RAI-MDS Coordinator is the professional responsible for ensuring the MDS is completed, accurate and submitted on time. The Resident Assessment Instrument (RAI) minimum data set (MDS) is an assessment the long-term care home must complete for each resident on admission, quarterly and annually or in the event of a significant change in condition. The purpose of the RAI-MDS is to assess how well residents in long-term care home’s function, and what interventions are being provided to support them. Reports toManager of Resident Care (MRC)Qualifications -Current regulated health care practitioner, RN or RPN in good standing with the College of Nurses.-Current knowledge of MOHLTC and College of Nurses Standards.-Minimum of 3 years clinical, project leadership or management experience.-Well-developed facilitation and presentation skills in keeping with Adult Education best practices.-Computer proficiency in Microsoft Word, Excel, Outlook, PowerPoint, Internet Explorer and PointClickCare.-Excellent planning, implementation and time management skills.-Understands clinical practice guidelines and current resident care practices.-Demonstrated success leading new initiatives and proven ability to get things accomplished.-Team oriented with ability to work with and relate to interdisciplinary Team Members in the Home.-Excellent communication, organization and problem-solving skills.-Clear aptitude for detail-oriented work.
